Re-entry Information

If you are an EMS First Responder, EMT-Basic, or EMT-Intermediate and you were unable to renew your license because you did not complete any or some of the renewal requirements, you will have to

1. Complete a Registration for Training/Licensure Application front and back.  This form is a scantron form and can not be downloaded. To obtain this form, please contact the EMS Bureau at (505) 476-8200 and the form will be mailed to you.

2. Attach a copy of a current refresher course completion certificate at your licensure level (completed within the last 9 months), or alternatively, submit proof of bureau approved CE credit that adheres to the refresher course blueprint at the appropriate level (also completed within the last 9 months).

3. Attach a copy of a current healthcare provider CPR card.

4. Attach a check, money order or PO (NO cash accepted) for the appropriate re-entry fee:

EMS First Responder  $ 60.00

EMT-Basic $ 90.00

EMT-Intermediate $120.00

You have two years from the date of license expiration to complete the re-entry process to include state testing. Once we have received your complete re-entry packet Ute Fennicks will schedule you for the next available state test site to complete your NM EMS state exam. A confirmation letter will be mailed to you. You will receive two (2) testing attempts to regain NM EMS licensure. If you are unsuccessful in completing the NM state exam in two attempts, you have to repeat an entire EMT course to be again eligible for NM EMS state testing.
