Initial Licensing - EMT- Paramedic
In-State Applicants for EMT- Paramedic - (Applies to all graduates of New Mexico training institutions)
The following documents are required:
- Completed application form (Download Here)
- $85.00 fee payable to the EMS Bureau (check, PO, or money order)
- Copy of current ACLS card
- Copy of current BLS CPR card*
- Copy of National Registry Paramedic certification**
- Copy of Paramedic course completion certificate
Out-of-State Applicants for EMT- Paramedic
The following documents are required:
- Completed application form (Download Here)
- $170.00 fee payable to the EMS Bureau (check, PO, or money order)
- Copy of current ACLS card*
- Copy of current BLS CPR card**
- Copy of current state Paramedic license
- Copy of National Registry Paramedic license***
- Copy of Paramedic course completion certificate****

- American Heart Association “ACLS”, “ACLS EP”, “HeartCode ACLS”
- Emergency Care and Safety Institute “ eACLS”
- American Safety & Health Institute “ACLS”
- Military Training Network “ACLS”
** The following are approved CPR courses:
- American Heart Association "BLS Healthcare Provider"
- American Red Cross "CPR/AED for the Professional Rescuer"
- Emergency Care and Safety Institute “ Professional Rescuer CPR”
- National Safety Council "CPR and AED"
- Medic First Aid “Basic Life Support for Professionals (BLSPRO)”
- American Safety & Health Institute “CPR for the Professional Rescuer”
- Military Training Network “BLS Healthcare Provider”
- EMS Safety Services: CPR/AED for the Professional Rescuer
*** If you possess a state paramedic license, but do not have National Registry certification at the Paramedic level, you will be issued a 6-month temporary license until National Registry certification is attained. This license is void should you fail any portion of the National Registry exam.
****Applicants must successfully pass an EMT-P examination administered by the EMS Bureau.
